NeoHealth Primary Care Network, a network of six GP practices in West London, has commissioned Aide Health to support its population living with long-term conditions.
Serving around 30,000 patients, the PCN will start by offering the digital self-management platform to those with type 2 diabetes and hypertension at Exmoor Surgery and Golborne Medical.
NeoHealth has previously been honoured with the ‘Best PCN Newcomer of the Year’ award in recognition of its innovation and commitment to improving patient outcomes, including projects to enhance care for patients with chronic obstructive pulmonary disease (COPD).
Dr Yasmin Razak, GP partner and GP Trainer at Golborne Medical, said:
We are continuously looking to develop new ways of working with people who frequently visit their GPs and improve our ability to prevent unnecessary emergency hospital admissions. Looking at new solutions for long-term condition management is an important part of that work. Dr Razak is also the lead for Interprofessional Education for West London, heading the RIPEN research & training hub, focusing on improving the care of long-term conditions.
Aide Health, now in four primary care networks and available to approximately 300,000 patients, is a digital platform to help improve a patient’s capability for self-management, an approach shown to reduce utilisation in both primary and secondary care. Today, Aide has pathways to support people with asthma, type 2 diabetes, hypertension and COPD. A pathway for chronic kidney disease will be available in Q2 of this year as Aide Health expands its multi-morbidity offering.

On average, a person living with long-term conditions may have 4-8 conversations with their healthcare professional per year. The Aide platform, driven by conversational AI, has demonstrated that patients have an average of 4-7 conversations per day, providing ongoing support between consultations and reviews. These conversations are designed to help with medicine optimisation, capture monitoring such as blood pressure, mental wellbeing and symptom severity, and improve health literacy. This expansion to London comes shortly after Aide Health was one of 24 companies selected to join the prestigious Google for Startups ‘AI for Health’ programme.
