Health Data Consent

1. I (“you”, “me”) understand that Aide Health Limited (the “Aide” “we”, “our”) will collect, use and process my health data, including, but not limited to, medical history, prescriptions, diagnosis, treatments, conditions, health metrics (such as symptom severity and blood pressure) and other health data  that I provide Aide (“Health Data”) to the extent necessary for Aide to provide its digital health platform (including health plan) (the “Platform”) available via a mobile application and to access and use Aide’s services via such Platform to better understand and manage my long-term health conditions, including provision of relevant customer and technical support services in connection therewith (the “Services”). I understand that if I do not agree to these uses of my Health Data then I may not be able to use the Services and Platform.
2. Aide respects your privacy and is committed to protecting your personal data. Our website privacy policy contains more detailed information about our data processing activities (including about data security, data retention and lawful processing bases) and you should read that in conjunction with this consent form.  We will only use and process your personal data as set out in our data privacy policy, which can be accessed here www.aide.health/en-us/privacy-policy for more information.
3. I understand that I may withdraw my consent to the processing of my personal data by Aide at any time by emailing support@aide.health.
4. I understand that I may have a right to request erasure of my personal data (otherwise known as a ‘right to be forgotten’) by Aide at any time by emailing support@aide.health.
5. I understand that Aide may anonymise my personal data for future research, development and commercial purposes in a way that cannot be linked back to me.
6. For all questions related to the processing of personal data or Aide’s services, any complaint or any other question that you may have, please send an email to support@aide.health.
7. Our terms and conditions of the supply of our Services and our mobile app end user licence agreement will apply to you and you should read these in conjunction with this consent form.

I acknowledge that I have carefully read, understood and agree to the terms set forth in this consent form and give my consent to the collection, use and proposed processing of my Health Data to the extent necessary for Aide to provide the Services and to access and use the Platform.

Temple University Research Pilot — Supplemental Consent

This supplemental consent applies if you are participating in the Aide Health pilot offered through Temple University Health System ("Temple"). It is in addition to, and should be read alongside, the Aide Health Medical Data Consent Form above and the Aide Health Terms of Service.

About the research project

Aide Health Limited ("Aide") has entered into a Data Transfer and Use Agreement with Temple to support a research project titled "Leveraging a novel, conversational AI tool to support Type 2 Diabetes management in the primary care setting" (the "Project"). The Project is led by researchers at Temple and has been reviewed by Temple's Institutional Review Board.

What data Aide will share with Temple

If you consent below, I understand that Aide will share the following data about me with the Temple research team:

• my name;
• my telephone number;
• my email address; and
• my application usage data, including the number of conversations I have with the Aide app, the frequency of those conversations, and the content of those conversations.

I understand that the content of my conversations with the Aide chatbot may include information I have shared about my health, medication, symptoms, lifestyle and personal circumstances, and that this content will be readable by named members of the Temple research team.

How Temple will handle my data

I understand that:

• Temple will store my data in password-protected files and will assign me a subject ID;
• Temple will analyse the data in de-identified form and will only share aggregated findings;
• the link between my identity and my subject ID will be deleted at the end of the study;
• Temple may retain one archival copy of the data after the study ends, where required by law, institutional policy, or for research integrity purposes; and
• Temple's researchers will not contact me about the research without separate approval from Temple's Institutional Review Board.

Withdrawing my consent

I understand that:

• I may withdraw my consent to Aide sharing further data with Temple at any time by emailing support@aide.health;
• data already shared with Temple before my withdrawal may be retained and used by Temple in accordance with Temple's research protocol and applicable law, even after I withdraw; and
• withdrawing my consent to data sharing with Temple does not affect my separate rights in relation to the data Aide holds about me, which are described in the Aide Health Medical Data Consent Form above.

Research participation

I understand that:

• my participation in the Project is entirely voluntary, and I may decline to participate, or withdraw at any time, without giving a reason and without any effect on my access to care at Temple or my relationship with my clinicians;
• the principal investigator at Temple is Janet Lee, who can be contacted at Janet.Lee2@tuhs.temple.edu, and the Project has been reviewed and approved by Temple's Institutional Review Board; and
• for questions about my rights as a research participant, or to raise a concern about how the research is being conducted, I can contact Temple's Institutional Review Board at the contact details Temple provides to me.

Consent

I have read and understood this supplemental consent and agree to Aide sharing the data described above with the Temple research team for the purposes of the Project.
